MN Self-Directed Participant Network : We are a grass roots group made up of consumers, caregivers, and other interested parties. Our goals are to educate, provide input, assist in change, and sustain CDCS in MN

CDCS is Consumer Directed Community Supports, we believe this is a revoluntionary option that helps prevent or delay a transition into a nursing home, assisted living, group home, or an institution.

Many individuals with disabilities and older Minnesotan's say, "I'm not going into no nursing home, group home etc.. .. I want to stay living in my home/apartment (with family) and my neighborhood." This revoluntionary option: CDCS lets you do just that. You Decide. Your Help.

How to become certified Support Planner

Support Planner

Support Planner services can help a person decide what will work best for them. A Support Planner is a person who has been certified by the Department of Human Services as someone who understands the CDCS service, person-centered planning and the process of developing a support plan. If a person feels they need more professional help to develop their support plan, they can use the Support Planner to help them.

How can a Support Planner help me?

A Support Planner is able to:

• Provide information about CDCS and provider options

• Help develop a Community Support Plan

• Find services and supports

• Make arrangements for purchases and delivery of services and supports

• Help solve problems that may occur

• Help monitor the Community Support Plan (DHS-2925 or DHS-4166)

• Assist with recruiting, screening, hiring, training, scheduling, monitoring and paying support workers

• Provide and arrange for staff training specific to the person’s needs.
What are the requirements to be a Support Planner?

A Support Planner must:

• Be 18 years of age or older

• Be able to coordinate their Support Planner with required case management services

• Complete a training module of at least six hours duration developed and approved by DHS on person-centered planning in addition to information on the Vulnerable Adult Act and Maltreatment of Minors Act

• Successfully pass the initial Support Planner certification test

• Eligibility is maintained by taking and successfully passing the Support Planner certification test every two years thereafter

• Use the Community Support Plan or similar format that includes all of the information required to authorize CDCS.

Persons may require additional qualifications of their Support Planner, which should be written in their Community Support Plan.

We would like you to share your success story using CDCS. There is a success CDCS story template attachment (Scroll to bottom of page) that will help you write your story. Type your story in Microsoft Word and send to arcarcLm103@yahoo.com and I will add it to this page as an attachment. I would like to highlight a story in this section every two months. Send your stories in.

Pat's Success Story
Pat had just finished her Masters in education when she started having problems with her health. Friends thought she was just run down but this active mother and hockey player knew something wasn't right. She was soon diagnosed with Crohn's Disease and then Chronic Fatigue Syndrome. Pat's life style changed suddenly and dramatically. She struggled with work and family responsibilities and finding doctors who would listen to what she wanted. In 2001, Pat attended a training by Partners in Policy Making. This started her networking and she soon found about a waiver program. Once Pat enrolled in the waiver program and had some more choice in her services, she felt like she could "finally take a deep breath".
However, when Pat needed some nutritional supplements, she wasn't able to get them through her waiver program. Someone suggested she join Minnesota's Cash & Counseling project, CDCS. At first, Pat wasn't sure if she wanted the increased responsibility of the program, but after enrolling she found that it "opened my eyes to what healthcare can be". After dealing with workers that she wasn't always happy with or struggling to fit her holistic healthcare approach into a medical model, CDCS was finally offering her the opportunity to get what she needed. Pat says she "felt helpless a lot of the time, now I'm thinking ahead about what needs to be done and who I can call to help".
Pat uses her budget to hire workers and purchase goods. Her son helps with some chores around the house. She uses a church group to do household cleaning and yard work. Pat has a partnership with a student from the Middle East who does household repairs for her and who she helps learn English. In addition, Pat purchases nutritional supplements needed for her special diet. She also has a woman who helps her with her paper work and financial matters. She has formed a team with her flexible case manager and county case manager. They learn from each other and work together to make CDCS work.
Pat has joined a state advisory group and is always finding people to talk to about enrolling in CDCS or becoming a flexible case manager. "Rather than seeing myself as a sick person, I know see myself as a pioneer. I'm on a mission".

Driving directions and parking details are on the DHS Web site. Following brief program updates, participants will form small groups to discuss innovations and suggest program improvements. Please feel free to pass the word to others working with CDCS services. I hope to meet you there!

Fiscal support entity services

A fiscal support entity offers a range of support services that allows a person to have as much choice as they want in employing, managing and paying for their services and supports. If a person chooses CDCS as a service option, the person must choose a fiscal support entity (FSE) to help them with employer-related responsibilities. A fiscal support entity should not in any way:

• Assist in writing the Community Support Plan

• Limit or restrict service or support providers
What types of assistance can a fiscal support entity (FSE) offer?

Fiscal support entities may provide a range of services as long as they provide payroll assistance at the minimum.

• Fiscal Conduit Model - FSE is the bill payer; person is the employer

• Payroll Model - FSE is the bill payer, helps with paperwork and provides education to the person; person is the employer

• Agency with Choice Model - FSE is the bill payer, helps with paperwork, provides education to the person and is the employer
