MN Self-Directed Participant Network : We are a grass roots group made up of consumers, caregivers, and other interested parties. Our goals are to educate, provide input, assist in change, and sustain CDCS in MN

CDCS is Consumer Directed Community Supports, we believe this is a revoluntionary option that helps prevent or delay a transition into a nursing home, assisted living, group home, or an institution.

Many individuals with disabilities and older Minnesotan's say, "I'm not going into no nursing home, group home etc.. .. I want to stay living in my home/apartment (with family) and my neighborhood." This revoluntionary option: CDCS lets you do just that. You Decide. Your Help.
